Petition Process
Complete this form and attach required documentation to request Administrative Withdrawal (Grade of "W") for courses after a grading period has ended.
G/PPS No. 02.08 (08.01) states that Deans may grant Administrative W's for students that present documentation of extremely extenuating circumstances that prevented them from successfully completing coursework.
- To withdraw from all courses within a given semester, appeal to the College Dean for your major—this form is only for majors in the College of Applied Arts.
- If you are requesting an Administrative Withdraw for a single course housed outside of the College of Applied Arts, you will want to contact that college in question.

Justification for Petition
Write a detailed explanation of the "extremely extenuating" circumstance that you would like to have considered for the Administrative W's.
**Your grades must be posted on your transcript in order to complete this form. If your grade has not been posted on your transcript, you must wait until grades are posted.
