Academic Suspension Appeal Appointment Request Form

The academic suspension appointment request form should only be completed if you have been placed on academic suspension and have a major housed within the College of Applied Arts. The deadlines to complete this form are found below:

  • Spring 2025: January 3, 2025 at 12:00PM

The College of Applied Arts Academic Advising Center will contact you after completion to schedule your academic suspension appeal appointment (Contact will be made within 24-48 hours of completion Monday-Friday). 

At your advising appointment you will be provided with GPA calculations and the appeal form that the College of Applied Arts Appeal Committee will review.
